7/5/2023 EnchantaVaughn Approved (AM) Upon arrival to the summer camp setting, students have reported client has been saying inappropriate words in class (e.g. profanity they did not want to repeat). When asked about it, client states this accusation is not true. No further redirection was needed. Client behaviors will be monitored closely. QMHP gave a lecture to client and peers to make sure everyone is aware that office setting is not a playground and that they are responsible for participating in the learning opportunities and must respect each other. Client was observed listening and taking in the feedback without getting upset or frustrated. Client was given a choice to work independently or as a group during class activities. Client was seen having difficulty in the academic setting (e.g. not being able to rhyme with simple words or identify different sounds). (PM) Client was a little confused about the directions given and tried to say whatever came to mind. Client was encouraged to listen carefully and think before he responds. Client was educated with a strategy to help him understand his work better. Client is reminded to practice and study at home on a consistent basis. (EOD) Client was observed on task and being cooperative with his peers. Client was able to reframe from impulsive, hyperactive behaviors until dismissal, achieving goals and objectives. (AM) QMHP reviewed previous days activities. QMHP inquired about evening and morning and how client was feeling. QMHP asked if he needed help with anything. QMHP encouraged client to have a good day. (PM) QMHP monitored for acquisition of objectives. QMHP encouraged client to stay in class and avoided talking while others are talking. QMHP encouraged client to be the example and raise his hand before leaving his assigned area. QMHP encouraged client not to follow others that he see are doing wrong. QMHP intervened to assist in explaining behaviors to client. (EOD) QMHP reflected with client on their day, the good and the bad experiences. QMHP what would you do different tomorrow. QMHP encouraged client to have a good evening. QMHP encouraged client that tomorrow is a new day. QMHP praised client for his commitment. QMHP processed client positive responses. QMHP assisted client with recognizing his own personal strengths and how to apply them to reach his goals and objectives. (AM) Client talked about the fun things he learned yesterday. Client said he woke up feeling refreshed and ready to have a good day. Client smiled and hugged QMHP. Client communicated by nodding and waving. Client was able to raise his hand appropriately and was given permission to see his counselor. (PM) Client was displaying respectful, non aggressive behaviors in lunch. Objectives were being met at this time, considering stressful and loud environment. Client avoided distractions in group setting and was seen paying attention in class. Client exhibited classroom management skills by staying in his seat and contributing to discussion. Client seem to have a big influence on peers and does not follow behind those who he see are doing wrong. Client acknowledged interventions and verbalized understanding. (EOD) Client engaged in reflective thinking. Client communicated the do's and don'ts. Client was in a more positive mood. Client said "thank you". Client enjoys being praised, as evidenced by his happy expressions. Client was able to identify all the good things that happened and see the role he plays in each situation. Client applies himself when utilizing his strengths and focusing on his goals and objectives.
